What are your hours of operation?

We are open Monday, Wednesday, Thursday and Friday from 8:30AM to 5PM. On Tuesday’s we stay open from 8:30AM to 7:30PM.

Where are you located?

We are located at 1900 Second Avenue (99th Street) in Manhattan. For all programs, enter through the Mental Health Pavilion of Metropolitan Hospital Center at the 2nd Avenue entrance.

For primary care, ECDC, and FESP, take Elevator Bank E to the 9th floor. For our ECP, MAP, and PAAM programs, take Elevator Bank 6 to the 12th floor. 

How do I make an appointment?

If you're interested in primary care, then give us a call at (212)360-7893 during our hours of operation. If you happen to call us when we are not open, just leave a message and we will call you back within one business day.

If you are interested in our FESP program, then come in during our intake hours, Monday – Friday from 9AM-10:30AM. For our ECP, MAP, and PAAM programs, our intake hours are Monday – Friday from 8:30AM – 11:00AM.

How long do I have to wait for a doctor’s appointment?

That can vary. We make every effort to accommodate you as soon as possible.

Do you take walk-ins for primary care?

We do accept walk-in appointments. We do recommend that you call our office before you come in, but if you can’t, then we will make every effort to meet your medical needs when you come in.

What should I bring with me to my doctor’s appointment?

Please bring a photo ID, your insurance card, and if it’s your first time, then bring this completed form with you (we have extra copies at the office, if you can’t fill it out before).

What insurances do you accept?

We accept the following insurance plans:

Aetna, Affinity Health Plan, AmidaCare, Beacus Health Strategies, Blue Cross Blue Shield, Cigna, Fidelis Care, EmedNY, Healthfirst, HealthNet, Medicare, Metroplus, Oxford, United Health Plus

What if I don’t have any insurance?

No problem. We have a sliding scale fee. You just need to bring in proof of income.

What if I don’t have proof of income?

If you don’t have any proof of income, then one of our staff members will discuss other options with you when you come in.  

Do I need a referral to receive services?

No referrals are needed, unless your insurance says so.

How do I access my medical records?

If you need access to your medical records, you will need to come in and sign the following consent form in person and bring a photo ID with you. We do this to ensure your confidentiality. After that, it usually takes 7-10 business days to get it to you.

Do I need to be a patient of primary care to receive any of your other services, including parent education, counseling, etc?

Not at all. Though we would love for you to get all your services with us, we understand that it might not be feasible for you. All of our services of available to you, whether you choose to receive your primary care with us or not.

I’m confused, is ECDC part of CCHP?

Yes it is. ECDC was started by the Junior League of New York, but we are all one big happy family!
