Jozefa came to CCHP in 2010 after graduating magna cum laude from the University at Buffalo School of Nursing. While at CCHP, she has dedicated much of her time to increasing patient education on Hypertension, Diabetes and Asthma prevention and management, and is currently developing both a Hepatitis C Treatment and Asthma Management program.
Director of Human Resources
Kristina is an accomplished human resources professional with a focus in human resources administration in the fields of education, social services, non-profit, and healthcare. She holds a bachelor’s in Business Administration and an MBA in Human Resources Management from La Sierra University and is a Certified Professional in Human Resources Kristina is a member of the Society for Human Resource Management - New York Chapter and the Association of Healthcare Human Resource Administrator of Greater New York.
Denise M. Fortino
Program Supervisor, Extended Care Program
Denise has been Site Administrator at CCHP since 2006 and the ECP Supervisor since 2003, after serving as a Senior Counselor for both the MAP and PAAM programs. Before coming to CCHP, Denise was the publications manager at Lenox Hill Hospital and the health and articles editor at Harper’s Bazaar Magazine. She has a doctorate in psychology from Saybrook University, master’s degrees in Forensic Psychology from John Jay College of Criminal Justice and English literature from Fordham University, and was a pre-doctoral student in Theater Arts at Columbia University.
Director of Development and Communications
Michelle came to CCHP after receiving her master's in Urban Policy Analysis and from the Milano School of International Affairs, Management and Urban Policy at the New School. She comes to us with over seven years’ experience in development, communications and program management working in the nonprofit sector. Michelle obtained experience in community health while working at the William F. Ryan Community Health Network in New York as a Planning Associate and Program/Community Relations Coordinator.
12th Floor Site Adminstrator
Rachel joined CCHP in 2008 as a Senior Counselor. She has since been an Intake Coordinator and now a Program Supervisor, specializing in addictions services, mood disorders, and children and adolescents. Rachel received her under graduate degree at Binghamton University's School of Education & Human Development and her Master’s in Social Work from the University of Pennsylvania's School of Social Policy & Practice.
Before Perly came to CCHP, he worked as a Contract Coordinator for the Visiting Nurse Service of New York. He has a bachelor’s degree in business, management, and finance from Brooklyn College.
Program Supervisor, Family and Employment Services Program
Audrey has worked in various capacities in the Chemical Dependency Outpatient Program as a Senior Counselor, HIV Case Supervisor and now Program Supervisor. Audrey received her Bachelors Degree at SUNY New Paltz with a major in Psychology and a duel minor in Disaster Mental Health and Evolutionary Studies. She went on to complete her Masters Degree in Social work at Hunter College. Prior to CCHP, she worked for an ACT team providing psychotherapy and case management and spent a year in Guatemala working with people in indigenous communities. She is fluent in Spanish.
Clinical Supervisor, Family and Employment Services Program
Beth is a clinical social worker, who first came to CCHP as a Senior Counselor working with clients through individual and group therapy. She earned her CASAC and specializes in addictions therapy. Beth received her undergraduate degree at Boston University and her master’s in social work from Hunter College.
Director of the Early Childhood Development Center
Becky started out as a volunteer with our ECDC program, eventually becoming its director in 1989. She is the co-author of ECDC’s three volume series on the first three years of life. She serves on the boards of Arts Connection, the Gateway School and Berkshire Choral Festival. Prior to joining ECDC, Becky earned a Masters of Educational Psychology from the University of Kansas, and did an extensive amount of charity work in Botswana.
Program Supervisor, Pregnant Addicts and Addicted Mothers Program
Liliana first came to CCHP in 1997. She has over 26 years’ experience working as a substance use specialist. She has a master’s degree in social work from Fordham University, and is a certified alcohol and substance abuse counselor. She was also a clinical psychologist in her native country, Peru, before coming to CCHP.
