CCHP’s Primary Care Program utilizes a model that encompasses the medical, social, emotional, and child development concerns of families. By emphasizing on health education, prevention and early treatment of illnesses our trained staff is able to intervene before minor issues become life threatening conditions.

To accommodate your needs, we offer a supervised playroom for your children (free of charge). Our experienced playroom teachers will watch and engage with your children while you are at your doctor’s appointment.
Our doctors are board certified/eligible in Internal Medicine, and emphasize preventive care and health maintenance. Keeping in line with best care practices that are both creative and current, we provide personalized, high-quality care to patients over 18 years of age.
Our board certified pediatricians are dedicated to delivering outstanding family-centered care to all patients from infancy to 18 years of age. If your child needs vaccines, is sick, or is in need of routine health examinations, our pediatricians are committed to helping your children thrive and stay healthy.
In addition to providing medical care, our Pediatrics Department participates in the Reach Out and Read program. To promote early childhood literacy, every patient between the ages of six months through five years old gets a developmentally-appropriate book at the time of their regular pediatric visit.
Prevention and Treatment of HIV/Hepatitis C/Other Infectious Diseases
CCHP offers comprehensive services for HIV, Hepatitis C (HCV), and other infectious diseases, including primary medical care and medical case management. These services offer supportive counseling and education, coordinate referrals, assist with insurance matters, and connect patients to eligible housing programs. We also offer substance use treatment services which include behavioral health and individual and group counseling to target the co-occurring disorders that many people living with HIV, HCV, or other infectious diseases struggle with every day.
We understand that it is just as important to learn about and understand your health, as it is to attend regular check-ups. Our experienced staff lead regular health education groups to patients on diabetes, HIV/AIDS, asthma management, hypertension, and women’s health.
