Social worker - PAAM
CCHP is seeking a social worker for its PAAM program, to be part of an interdisciplinary staff caring for individuals and families with substance use and related problems.
The Pregnant Addicts and Addicted Mothers Program, PAAM, was among the first in the nation to offer truly comprehensive care to heroin-addicted women and their families. The overall goals are for each woman to curtail substance abuse; receive physical and emotional health care; improve economic self-sufficiency; be an able parent; and, if pregnant, deliver a healthy baby. An individual service plan is developed to meet the specific needs of each woman. Women are encouraged to enroll with the knowledge that services are designed specifically for women.
Responsibilities include individual counseling, group therapy, treatment planning, vocational services, and case management.
Qualifications
- At least one year of experience
- Requires a valid NYS LCSW or LMSW degree
- Bilingual in Spanish and English preferred (but not required)
Position is part of team practice model which also includes internal medicine, pediatrics, nursing & psychiatric services. Academic affiliation with New York Medical College.

CCHP is seeking an on-call Play Teacher to supervise and to engage groups of children (Aged 0-11 years old) in age appropriate activities while parents attend medical/ counseling appointments and parenting classes. This position offers an opportunity for gaining skills and experience in early childhood education and development.
Essential Duties and Responsibilities
- Planning age-appropriate activities and coordinating plans with head play teacher and staff
- Engaging children in play that is enjoyable with minimum stress
- Functioning as a child's first teacher outside the home
- Helping shape future social and emotional development.
Qualifications
- Applicant should be enrolled in an undergraduate or graduate school program.
- Applicant should have experience working with young children 0-11 years of age.
- Must have the following availability: Mon: 9am - 5pm, Tues: 9am - 1pm, occasional Wed evening: 5pm - 8pm, Thurs: 9am - 1pm.
Applicant should have an understanding of child development issues, be creative, flexible, dependable, and must enjoy very young children.

Social Worker - FESP
CCHP is seeking a social worker for its FESP program, to be part of an interdisciplinary staff caring for individuals and families with substance use and related problems.
The Family and Employment Services Program, FESP, helps individuals and families with children, particularly families whose parenting is made more difficult by substance use. The general goals are to overcome addiction, stabilize family relationships, improve overall health and mental health status, and expand employment options.

social worker - MAP
CCHP is seeking a social worker for its MAP program, to be part of an interdisciplinary staff caring for individuals and families with substance use and related problems.
MAP is a methadone-to-abstinence program which offers outpatient help to individuals whose primary substance use is opioids. Certified staff provides program participants with ongoing care, including methadone treatment to stabilize their substance use, counseling services to address related problems, and medically supervised withdrawal when appropriate.
Responsibilities include individual counseling, group therapy, treatment planning, vocational services, and case management.
Qualifications
LMHC/LMSW required
At least one year of experience
Bilingual in English and Spanish preferred
