Your New Medical Home

At the Center for Comprehensive Health Practice (CCHP), our mission is to provide accessible community-based treatment that integrates primary health care, substance use and behavioral health services for all children, adolescents, adults and families residing in East Harlem and the greater New York Community.

We are a patient-centered, family-focused community health center dedicated to the health and wellness of our patients and the communities we serve. Our medical providers and staff are committed to comprehensive, personal healthcare centered on you; partnering with you to ensure that we meet all of you and your family’s needs.

YOUR PERSONAL PRIMARY CARE PROVIDER (PCP)

The relationship between you, your PCP, and the care team (clinic staff) is the driving force behind a Patient-Centered Medical Home (PCMH). Your PCP will provide medical care that is right for you based on your needs and evidence-based guidelines shown to improve your health. You will be asked to designate one of our physicians or nurse practitioners as your PCP.

YOUR CARE TEAM

Your PCP will direct the care team to coordinate your care based on your preferences and needs. To improve efficiency, the team will plan for your appointment by reviewing your medical chart for up-to-date forms, check for recent testing, and insure you are notified of results in a timely manner. The care team will work with you and any community partners to coordinate your healthcare across all specialties, including behavioral health. Your care team will make sure your appointment is timely, provide the specialists with your care summary before your appointment, and obtain any specialist recommendations after your appointment.

Your Health  

In return, we ask that you continue to be an active participant in your healthcare. We ask you to take charge of your health by responsibly managing and monitoring aspects of your care. Please complete the medical history and keep us informed of your medications, visits to specialists, medical history, health status, recent test results, self-care abilities and information from recent hospitalizations, emergency room or urgent care visits. In addition, you can help by being prepared and on time for your appointment and brining all of your medication bottles along with you. Follow the care plan our team has developed for you, and let your PCP know when you need assistance in doing so. We are here when you need us. You can call our office, or follow the guidelines for reaching us during non-business hours before going to the Emergency Room in a non-life threatening situation

Our PCMH is committed to providing same day appointments and offering expanded hours to meet your needs. We will use our electronic medical record, eClinicalWorks, to support best care practices to meet your needs and the needs of our community. Additionally, we are able to contact you via electronic reminders for appointments, preventative, and chronic care services.


PRIMARY CARE PROVIDERS


Mariely Fernandez, MD

Dr. Fernandez is CCHP’s Chief Medical Officer and a pediatrician. She has been at CCHP since 2007. Dr. Fernandez graduated from the Ross University School of Medicine in 2003 and completed her residency at the Steven and Alexandra Cohen Children’s Medical Center. She is board-certified in Pediatrics and Addiction Medicine. She has a special interest in childhood obesity and community pediatrics. Dr. Fernandez is bilingual in English and Spanish.

Anna Renkiewicz, MD

Dr. Renkiewicz came to CCHP in 2012. She graduated from the Medical University of Gdansk in Poland in 2003 and completed her residency at Lutheran Hospital in Brooklyn, NY.

Dr. Renkiewicz is board-certified in Internal Medicine and has a special interest in Hepatitis C, the neurophysiology of addiction, and holistic medicine. Dr. Renkiewicz is bilingual in English and Polish.

Sharon Chu, MD, MPH, AAHIVS

Dr. Chu came to CCHP in 2018. She graduated from SUNY Downstate Medical Center with her medical degree and master’s in public health. She completed her residency training at Lancaster General Health in Lancaster, PA. She is board-certified in Family Medicine. She is also certified as an HIV specialist by the American Academy of HIV Medicine. She has a special interest in HIV care and prevention, family and maternal health, preventive medicine, and addressing health disparity. She is multilingual in Urdu, Hindi, Spanish, and English.

Zobidatte Moussa, MD

Zobidatte Moussa, MD joined CCHP in May 2024. He is board-certified in Internal Medicine and has been providing primary care in NYC for more than 20 years.  Dr. Moussa is a graduate of the Technion School of Medicine in Israel and completed his residency program at Maimonides Medical Center in 1992.  He has a special interest in cardiac ailments. He is fluent in English, Arabic, and Hebrew.

Judy M. Holder, MSN, RN, FNP-VC

Judy Holder is a Nurse Practitioner at CCHP, where our patients appreciate her compassionate and steady nature. She has been a Registered Nurse since 2010 and received her Master of Science from Lehman College. Prior to CCHP, Nurse Holder worked at New York Presbyterian (NYP) coordinating their annual influenza campaign, then transferred to NYC Health + Hospitals at the beginning of the pandemic, working on the COVID-19 testing team doing critical testing and vaccination work.

Delcarmise Napoleon, FNP

Delcarmise Napoleon has decades of experience in diagnosing, evaluating, and treating acute and chronic medical conditions as a nurse practitioner. She has a history of providing compassionate care, assisting physicians, and managing plans of care. She brings her diverse experience and her holistic care approach to the CCHP family. Nurse Napoleon has practiced in New York and Haiti. She has a Master in Science of Nursing Family Medicine & Bachelor of Science from Dominican University.

Our providers utilize evidence-based medical practices to ensure that you are receiving the most up-to-date and highest quality medical care.  In addition, our nursing staff provide both one-on-one and group instruction for a variety of health care issues such as diabetes, hypertension, HIV, etc., to individuals, family members and other caregivers.  Lastly, CCHP clinicians promote health self-management through education and other supports.

The Early Childhood Development Center (ECDC) uses a specially developed curriculum to teach parents of children up to three years old the principles of childhood development and provides on-going support and guidance through a facilitated group setting.

Registration information is collected during your first visit to our office.  This information and any other information given to our office will be kept in the strictest confidence. At each visit, you will be asked present your insurance card and photo identification. Additionally, you will need to verify:

Payment

At check-in, your account is reviewed for any outstanding balances and co-payments. Payment in full is expected at the time of service. You can pay by check, credit/debit card, or cash. Please make checks payable to CCHP. You will receive a receipt for each paid office visit.  Please be aware that there may be additional charges for tests such as blood work, PPD, EKG, and vaccinations (not for children) if not already covered by your insurance provider.

If you are unable to provide full payment at the time of service, please see our Front Desk for assistance. A statement will be mailed at the end of the month. We realize that special situations may arise that require alternative payment plans. In these circumstances, please let us know so an individualized plan can be arranged with our Accounts Manager.

CCHP provides equal access to and the same quality of care for all patients regardless of their ability to pay.   

 
Office Processes:


Hours of Operation   

Our office is open Monday, Wednesday, Thursday and Friday from 8:00 am to 5:00pm and Tuesday from 8:00am to 6:00pm.  Additionally, we offer same day appointments Monday-Friday both in person and via telehealth. On-call staff is available for urgent calls after hours. Please see additional details below.

 

Days Off

If your PCP is out of the office or unavailable during your requested appointment time, you will be given the option to see another PCP. Our healthcare providers collaborate daily, as part of a care team, and are fully prepared to manage each patient’s individual care needs. It is important to become familiar not only with your PCP, but also with each health care provider on staff at CCHP.

 

Appointments

You can arrange an appointment by calling our office at 217-360-7700. To effectively manage your care, please inform your PCP of all medications you are taking, any drug allergies or side effects, visits to other health care clinics, such as specialist visits or hospitalizations, changes in your medical history or health status, results on any outside lab or diagnostic testing, and updates to your self-care needs.

We also offer same day appointments both on-site and via telehealth. Please see our telehealth policy for additional information regarding telehealth visits. We recommend calling our office to schedule your appointment whenever possible.

For your convenience, we recommend that you request appointments for physical examinations and routine checkups well in advance. Other routine appointments are scheduled as available.

Cancellations

If you are unable to keep a scheduled appointment, we ask that you notify our office as promptly as possible.

If you arrive more than 20 minutes late for an appointment, you may be asked to reschedule or to wait until other scheduled patients have been seen. This avoids long waits and provides adequate time for a thorough visit.

After Hours   

If you or your family need emergency care or advice outside of office hours, CCHP has an answering service that can connect you to CCHP’s on-call medical provider. Simply call our main line at (212)360-7700 and you will be transferred to our after-hours answering service.

Coordination of Care   

As your Patient Centered Medical Home, CCHP will coordinate your care with any person, family member, caregiver, physician, organization, hospital, clinic, etc. in your Circle of Care.  Please keep us informed if you wish to add to those who can receive your medical information or remove anyone from your list.  It is particularly important that we are aware of any outside medical providers so that we can manage your care effectively.

Test Results

Calls regarding laboratory tests or x-ray results will not be returned until the results are available. Any abnormal results will be reported to you as soon as your PCP has reviewed them. A letter will be sent to you if all test results are normal. These results can be reviewed with your PCP at your next appointment. If you have activated your Patient Portal account, these results are also posted to your account for your review.

Medication Refills

Contact CCHP directly for a medication refill. Requests can be made by telephone or through your patient portal.  Please do so while you have at least 7 days’ worth of medication remaining. Please note that our medical providers may not automatically refill any medication if your last visit was more than three months from the date of the requested refill.

Medical Records

CCHP will gladly work with you to provide you or another doctor with your medical records. To obtain your medical records you must complete the Medical Records Request form in person and bring a valid photo ID. Medical Records are provided free of charge to other providers.  If you require a printed copy of your medical records, the fee is $0.75 per page for patients. Please allow for 7-10 business days for your medical records to be released. You can also access your patient portal at any time for medical information.

Outside medical records from other providers can be sent to CCHP by either fax (212)937-0928 or directly mailed to CCHP, attention Scarlett Luna.
