Registration

During your first visit to our office, registration information will be collected. This information and any other information given to our office will be kept in the strictest confidence. At each visit, you will be asked to present your insurance card and photo identification. Additionally, you will need to verify:

  • Address

  • Phone Number

  • Email

  • Insurance

  • Preferred Pharmacy

This is not an all-inclusive listing of each identifier captured and verified in the registration process. Please be prepared to provide additional information beyond the examples listed above.

 

PAYMENT

At check-in, your account will be reviewed for any outstanding balances, and any relevant co- payments. Payment in full is expected at the time of service. You can pay by check, credit/debit card, or cash. Please make checks payable to CCHP. You will receive a receipt for each paid office visit.  There is an additional charge for tests such as blood work, PPD, EKG and vaccinations (not for children).

If you are unable to provide full payment at the time of service, please make arrangements with our Front Desk before you meet with your provider. A statement will be mailed at the end of the month. We realize that special situations arise that require alternative payment plans. In these circumstances, we offer individualized, structured payment plans. Please discuss this with our Front Desk before your account is overdue.

Any other insurance, regardless of listed co-pays, is to be paid in full at time of service.

If you have insurance that covers office charges, you will be given a receipt upon payment to file for reimbursement to yourself.

If you do not have insurance, we offer an income-based sliding scale. We also can assist in obtaining affordable insurance coverage. Please contact the Front Desk for more information.

 

 
 

Patient Portal

 
 

The Patient Portal is a secure way for you to conveniently access your personal health information online. Through the portal you will be able to review your lab results, receive summaries of your provider visits, communicate with your provider and much more!

OFFICE PROCEDURES

HOURS OF OPERATION

 
 

Our primary care office is open Monday, Wednesday, Thursday and Friday from 8:30AM to 5:00PM and Tuesday from 8:30AM to 6:00PM. Additionally, we offer a limited number of acute care, Walk-In/Same-Day appointments Monday - Friday. On-call staff is available for urgent situations after hours. Please see additional details below.

DAYS OFF

If your primary care provider is out of the office or unavailable during your requested appointment time, you will be given the option to see another provider. Our healthcare providers collaborate daily, as part of a care team, and are fully prepared to manage each patient’s individual care needs. It is important to become familiar not only with your primary care provider, but also with each health care provider on staff at CCHP.

APPOINTMENTS

You can request an appointment by calling our office at 212-360-7700, by emailing us at primarycare@cchphealthcare.org, or through our Patient Portal. To effectively manage you care, at each visit, please inform your healthcare provider of all medications you are taking, any drug allergies or side effects, visits to other health care clinics, such as specialist visits or hospitalizations, changes in your medical history or health status, results on any outside lab or diagnostic testing and updates to your self-care needs.

We also offer a limited number of walk-in/same-day appointments for sudden illnesses, however, these fill up quickly. We recommend calling our office first thing in the morning if possible. For your convenience, we recommend that you request appointments for physical examinations and routine checkups at least 4 – 6 weeks in advance. Other appointments are scheduled as available.

 

 

CANCELLATIONS

If you are unable to keep a scheduled appointment, we ask that you notify our office as promptly as possible but no later than 24 hours prior to your appointment. 

If you arrive more than 15 minutes late for an appointment, you may be asked to reschedule or to wait until other scheduled patients have been seen. This avoids long waits for you and other patients, and provides adequate time for a thorough visit.

AFTER HOURS

If you or your family need emergency care or information outside of office hours, CCHP has an answering service that can connect you to your provider or CCHP’s on-call provider. You can call our main line at 212-360-7700 and you will be transferred to our after-hours answering service.

TEST RESULTS

Calls regarding laboratory tests or x-ray results will not be returned until results are available. Any abnormal results will be reported to you as soon as the provider has reviewed them. A letter will be sent to you if all test results are normal. These results can be reviewed at your next appointment. If you have activated your Patient Portal account, these results may also be posted to your account for your review.

MEDICATION REFILLS

You can contact CCHP directly or use the Patient Portal to request a medication refill. Please do so while you have at least 7 days’ worth of medication remaining. Note: our providers will not refill any medication if your last visit was more than three months from the date of the requested refill.

MEDICAL RECORDS

CCHP will gladly work with you to provide your or another provider with your Medical Records. To obtain your Medical Records you must complete the Medical Records Request form in person and bring a valid photo ID. Medical Records are provided free of charge to other providers and $0.75 per page for patients. Please allow for 7 - 10 days for your Medical Records to be released.

Medical Records can be sent to CCHP by either fax 212-937-0928 or mailed to CCHP.


PROVIDERS

The following information will help you select your preferred healthcare provider at CCHP. We practice together to provide a wide range of health care services for children and adults. Please see call the Front Desk at (212) 360-7700 if you have any questions or would like to switch providers.

Mariely Fernandez, MD, Chief Medical Officer

Dr. Fernandez has been at CCHP since 2007. Dr. Fernandez graduated from the Ross University School of Medicine in 2003 and completed her residency at the Steven and Alexandra Cohen Children’s Medical Center. She is board certified in Pediatrics and Addiction Medicine. She has a special interest in childhood obesity and community pediatrics. Dr. Fernandez is bilingual in English/Spanish.

Sharon Chu, MD, MPH, AAHIVS, Family Practitioner

Sharon came to CCHP in 2018. She graduated from SUNY Downstate Medical Center with her medical degree and masters in public health. She completed her residency training at Lancaster General Health in Lancaster, PA. She is board certified in Family Medicine. She is also certified as a HIV specialist by the American Academy of HIV Medicine. She has a special interest in HIV care and prevention, family and maternal health, preventive medicine, and addressing health disparity. She is multilingual in Urdu, Hindi, Spanish, and English.

Anna Renkiewicz, MD Internist

Anna came to CCHP in 2012. She graduated from the Medical University of Gdansk in Poland in 2003 and completed her residency at Lutheran Hospital in Brooklyn, NY. Dr. Renkiewicz is board certified in Internal Medicine and has a special interest in Hepatitis C, Addiction Medicine, neurophysiology of addiction and holistic medicine. Dr. Renkiewicz is bilingual in English/Polish.

Judy M. Holder, MSN, RN, FNP-VC, Nurse Practitioner
Judy is a Nurse Practitioner at CCHP, where our patients appreciate her compassionate and steady nature. Prior to CCHP, Judy worked at New York Presbyterian (NYP) coordinating their annual influenza campaign, then transferred to NYC Health + Hospitals at the beginning of the pandemic, working on the COVID-19 testing team doing critical testing and vaccination work. Judy has been a Registered Nurse since 2010. She started her RN career in outpatient case management then went to internal medicine at Lincoln Hospital before working on the vaccination drive at NYP. Judy has practiced around various boroughs of New York, and has a Master of Science from Lehman College.  Her last NP assignment was on the COVID frontlines.

Delcarmise Napoleon, Nurse Practitioner

Delcarmise is a successful nursing professional with decades of experience in diagnosing, evaluating, and treating acute and chronic medical conditions. She has a history of providing compassionate care, assisting physicians, and managing plans of care. She brings exceptional critical thinking and decision-making skills to the CCHP family. Delcarmise has practiced in New York and Haiti. She has a Master in Science of Nursing Family Medicine & Bachelor of Science from Dominican University.
